OUT-PATIENT

New Patients

1. Step 1 : Please proceed to Reception Desk
2. Step 2 : Present government issued identification card (KTP / SIM)
3. Step 3 : Fill out the form with personal information, such as your current address,  telephone number, and the next of kin
4. Step 4 : You will be given a Rumah Sakit Abdi Waluyo Patient Identification Card
5. Step 5 : You will be given a queing number
6. Step 6: You will be directed to the waiting area

Old Patient

1. Step 1 : Please proceed to Reception Desk and show your Patient ID Card
2. Step 2 : Please mention your appointment shedule to the receptionist
3. Step 3  : You will be given a queing number
4. Step 4  :  You will be directed to the waiting area

General Information

• Insurance card and forms must be submitted to the Insurance Assist Desk for processing and confirmation with insurance company.
• Previous medical records, test results, referral letters or copies of follow-up instructions regarding medical treatments from other hospitals must be brought when consulting to Rumah Sakit Abdi Waluyo doctor.
• Prescription of medicines from attending physician shall be given to the hospital Pharmacy. The Pharmacist will dispense and explain the dosage and confirm expiration date of he medicines to patients
• All payments of hospital bills including consultation/professional fees of doctor, medicines, radiology and laboratory procedure must be processed only at the cashier.

For emergency cases, you may go directly to the hospital and proceed to the emergency section. Patient information will be taken by the emergency nurse.

IN-PATIENT

New Patients

1. Step 1 : Please proceed to Reception Desk

3. Step 3 : Fill out the form with personal information, such as your current address,  telephone number, and the next of kin
4. Step 4 : You will be given RSAW Patient Identification Card
5. Step 5 : Previous medical records, test results, referral letters or copies of follow-up instructions regarding medical treatments from other hospitals must be brought for examined by our doctor
6. Step 6  : The attending physician will give an order for admission or confinement.
7. Step 7  : Patient and/or a family member confirms the confinement at the reception desk
8. Step 8 : Receptionist will process the Admission Procedure and confirms the type of room to the patient and/or family member.
9. Step 9 : Patient will be roomed-in
10. Step 10 : The nurse-in-charge will put the bracelet to the patient’s wrist, showing the  patient’s full name, date of birth, and the name of the attending physician.
11. Step 11 : Management of treatment and medications will be closely monitored by the medical team in-charge.

General Information

• For safety reasons,  Patient Bracelet must be worn at all times while being admitted at Rumah Sakit Abdi Waluyo as long as you are treated
• For patients with insurance coverage, please inform the Admission Officer of your insurance and submit the card for confirmation processing.
• All payments of hospital bills including Doctor’s Consultation or Professional fees, laboratory and radiology procedures must be processed only at the cashier.
